Payment Info

We are in-network with Blue Cross Blue Shield commercial plans (not marketplace plans), Cigna, most Aetna plans and United Healthcare commercial plans (clinician specific). If utilizing insurance that Fit Mind Wellness is in-network with, you should expect to pay a co-pay or deductible dependent upon your insurance plan. When utilizing in-network insurance you will only be responsible for the contracted rates set by your insurance provider. Fit Mind Wellness will not issue additional charges. Payment Due at Service Time.

Cancellation Policy

If you do not attend your scheduled appointment and do not notify us of cancellation within 24 hours of the appointment, you will be charged an $100 fee.

Pricing

Private Pay Rates vary by clinician level and license.

  • Intern: $60/session
  • LAC: $100-$140/session
  • LPC: $125-$150/session

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.

Payment Terms

By booking an appointment, you are consenting to pay for services regardless of your insurance coverage. You are financially responsible for costs incurred when a claim is denied due to changes in, lapses, or exhaustion of benefits or termination or denial of coverage for any reason. The card you list on file will be charged for patient responsibility fees. You can change or update the card on file at anytime.
