Payment

Payments are due at the time of service.

We are in-network with Blue Cross Blue Shield plans, Cigna, most Aetna plans and United Healthcare (clinician specific). If utilizing insurance that Fit Mind Wellness is in-network with, you should expect to pay a co-pay or deductible dependent upon your insurance plan. When utilizing in-network insurance you will only be responsible for the contracted rates set by your insurance provider. Fit Mind Wellness will not issue additional charges for covered services through your in-network insurance.

Missed Appointment/Cancellation Policy

If you do not attend your scheduled appointment and do not notify us of cancellation within 24 hours of the appointment, you will be charged an $100 fee.

Financial Responsibility

By booking an appointment, you are consenting to pay for services regardless of your insurance coverage. You are financially responsible for costs incurred when a claim is denied due to changes in, lapses, or exhaustion of benefits or termination or denial of coverage for any reason.

Private Pay Rates

Private Pay Rates vary by clinician level and license.

Intern: $50/session

LAC: $100-$125/session

LPC: $125-$175/session
